Why Blood Pressure Matters for DOT Physicals

Uncontrolled high blood pressure increases the risk of sudden incapacitation โ€” including heart attack and stroke โ€” which makes it a critical safety concern for commercial drivers. The FMCSA sets specific blood pressure thresholds that determine how long your medical certificate is valid.

Stage 2: The 3-Month Certificate

If your blood pressure is 160โ€“179/100โ€“109, the examiner may issue a one-time, non-renewable 3-month certificate. You have 3 months to get your BP under control with medication or lifestyle changes. When you return with BP below 160/90, you can receive a standard 1-year certificate. If you do not return within 3 months, your certification lapses.

Stage 3: Immediate Disqualification

A BP reading of 180/110 or higher means you cannot be certified that day. You must work with your doctor to bring BP under control before reapplying for your DOT physical. There is no temporary certificate at this stage.

Tips for Lowering Blood Pressure Before Your Exam

  • Limit sodium โ€” Reduce processed foods, fast food, and added salt.
  • Exercise regularly โ€” Even 30 minutes of walking daily can make a measurable difference.
  • Limit alcohol and caffeine โ€” Both raise blood pressure temporarily; skip coffee the morning of the exam.
  • Take your medication โ€” If prescribed antihypertensives, take them consistently. Don't skip doses.
  • Rest before your exam โ€” "White coat hypertension" (anxiety-driven spikes) is real. Sit quietly for 5 minutes before measurement.
  • Schedule your exam in the morning โ€” BP tends to be lower earlier in the day for most people.

Blood Pressure Medications and DOT Physicals

Most blood pressure medications are acceptable for CDL drivers. Common acceptable classes include ACE inhibitors, ARBs, beta-blockers, calcium channel blockers, and diuretics. However, some older medications (like certain alpha-blockers) may cause drowsiness, which can be flagged by the examiner. Always bring your full medication list and be prepared to discuss side effects.

Can You Retake Your BP During the Same Visit?

Yes. If your initial reading is elevated, a good medical examiner will allow you to rest for 10โ€“15 minutes and take a second reading. Many drivers experience white-coat hypertension. Bring documentation of your home BP readings if you monitor at home.
