What Is a DOT Physical?

A DOT physical (also called a CDL medical exam) is a federally required medical examination for all commercial motor vehicle (CMV) drivers operating in interstate commerce. The exam is governed by the Federal Motor Carrier Safety Administration (FMCSA) and must be performed by a licensed medical examiner on the FMCSA National Registry.

If you pass, you receive a Medical Examiner's Certificate (MEC, Form MCSA-5876) that is valid for up to two years. Without a current certificate, you cannot legally drive a CMV in the United States.

What Does the Examiner Check?

The exam covers seven key areas:

  • Vision โ€” You must have at least 20/40 acuity in each eye (with or without corrective lenses) and a horizontal field of vision of at least 70ยฐ. Color blindness does not automatically disqualify you.
  • Hearing โ€” You must be able to perceive a forced whispered voice from at least 5 feet with or without a hearing aid.
  • Blood Pressure โ€” Stage 1 hypertension (140โ€“159/90โ€“99) earns a 1-year certificate. Stage 2 (160โ€“179/100โ€“109) earns a 3-month certificate. Stage 3 (180+/110+) is disqualifying until controlled.
  • Urinalysis โ€” Tests for glucose and protein as diabetes indicators. This is NOT a drug test.
  • Cardiovascular & Neurological โ€” History of heart attack, stroke, or seizures is reviewed carefully.
  • Musculoskeletal โ€” Range of motion, strength, and any limb impairments.
  • Medical History Review โ€” You'll fill out Form MCSA-5875 listing conditions, medications, and surgeries.

How to Prepare for Your DOT Physical

1. Monitor Your Blood Pressure

High blood pressure is the most common reason drivers receive a shortened certificate or a temporary disqualification. In the weeks before your exam, check your BP regularly. If it's elevated, talk to your primary care doctor about short-term management. Avoid caffeine and strenuous exercise the morning of your exam.

2. Bring All Your Medications

Bring a printed list of every medication you take, including dosages and the prescribing doctor's name. Certain medications can raise concerns โ€” such as sleep aids, opioids, or antiepileptic drugs โ€” so the examiner may want to review documentation. Do not stop taking prescribed medications without consulting your doctor first.

3. Wear Your Glasses or Contacts

If you need corrective lenses to meet the vision standard, wear them to the exam. Your certificate will note "requires corrective lenses," and you must wear them while driving.

4. Get Enough Sleep

The examiner screens for sleep apnea risk. If you are overweight or have a large neck circumference, be prepared for questions. Having a recent sleep study (if you've been treated) can help avoid a referral.

5. Be Honest on Your Medical History Form

Falsifying Form MCSA-5875 is a federal offense. Examiners see thousands of these forms โ€” be straightforward. Many conditions can be accommodated with proper documentation.

Common Reasons Drivers Fail

  • Blood pressure above 180/110
  • Vision below 20/40 (uncorrected and uncorrectable)
  • Uncontrolled insulin-dependent diabetes (without a federal diabetes exemption)
  • History of epilepsy or seizures without an exemption
  • Certain heart conditions without clearance from a cardiologist

What Happens After You Pass?

Your Medical Examiner's Certificate is issued on the spot. Your results are also reported to the FMCSA National Registry within 24 hours, where your employer and state DMV can verify compliance. How Often Do You Need a DOT Physical?
