A DOT physical (also called a CDL medical exam) is a federally required medical examination for all commercial motor vehicle (CMV) drivers operating in interstate commerce. The exam is governed by the Federal Motor Carrier Safety Administration (FMCSA) and must be performed by a licensed medical examiner on the FMCSA National Registry.
If you pass, you receive a Medical Examiner's Certificate (MEC, Form MCSA-5876) that is valid for up to two years. Without a current certificate, you cannot legally drive a CMV in the United States.
The exam covers seven key areas:
High blood pressure is the most common reason drivers receive a shortened certificate or a temporary disqualification. In the weeks before your exam, check your BP regularly. If it's elevated, talk to your primary care doctor about short-term management. Avoid caffeine and strenuous exercise the morning of your exam.
Bring a printed list of every medication you take, including dosages and the prescribing doctor's name. Certain medications can raise concerns โ such as sleep aids, opioids, or antiepileptic drugs โ so the examiner may want to review documentation. Do not stop taking prescribed medications without consulting your doctor first.
If you need corrective lenses to meet the vision standard, wear them to the exam. Your certificate will note "requires corrective lenses," and you must wear them while driving.
The examiner screens for sleep apnea risk. If you are overweight or have a large neck circumference, be prepared for questions. Having a recent sleep study (if you've been treated) can help avoid a referral.
