Please read these Terms of Service ("Terms") carefully before using the website and ordering portal operated by Doctors Place Inc. ("Doctors Place," "we," "us," or "our") at portal.dot-physical.net (the "Service").
By accessing or using our Service, you agree to be bound by these Terms. If you do not agree to these Terms, do not use the Service.
Doctors Place Inc. provides an online ordering platform for occupational health services including, but not limited to: DOT physical examinations, pre-employment and random urine drug screens, breath alcohol testing, observed collections, lab work, vaccinations, and DOT random testing consortium enrollment.
We coordinate services through a network of third-party collection sites, medical examiners, and laboratories. Doctors Place acts as the ordering and coordination platform; the actual medical examination and testing services are performed by independent licensed medical professionals and certified collection sites.
You must be at least 18 years of age to use this Service. By using the Service, you represent and warrant that you are 18 years of age or older and have the legal authority to enter into these Terms.
If you are ordering on behalf of an employer or company, you represent and warrant that you have the authority to bind that entity to these Terms.
All orders placed through our Service are subject to acceptance and availability. We reserve the right to refuse or cancel any order at our discretion.
Payment: Payment is due at the time of ordering. We accept major credit and debit cards processed securely through Stripe. All prices are in U.S. dollars.
Pricing: Prices are subject to change without notice. The price displayed at the time of your order is the price you will be charged.
Refunds: Orders may be refunded if cancelled before the service is rendered at the collection site. Once a service has been performed (e.g., a specimen has been collected or an exam has been completed), no refund will be issued. Refund requests must be submitted to occmed@doctors-place.com within 30 days of the order date.
DOT Physicals: DOT physical examinations are performed by FMCSA-registered medical examiners. Passing the exam and receiving a Medical Examiner's Certificate (MEC) is not guaranteed. The medical examiner's determination is final and is made independently by the licensed examiner, not by Doctors Place.
Drug Testing: Drug test results are processed by SAMHSA-certified laboratories and reviewed by certified Medical Review Officers (MROs). We do not control or influence test results. Positive results will be reported to your employer as required by DOT regulations (49 CFR Part 40).
Regulatory Compliance: Drug testing and DOT physical services are performed in accordance with applicable federal regulations including 49 CFR Parts 40 and 382. You are responsible for understanding the regulatory requirements applicable to your situation.
No Medical Advice: Nothing on this platform constitutes medical advice. Results and reports generated through our Service are for regulatory compliance purposes only. Consult a licensed physician for medical advice.
Employers who create company accounts ("Employer Accounts") are responsible for ensuring that all orders placed under their account comply with applicable federal and state employment laws, DOT regulations, and their own company policies.
Employers are responsible for obtaining proper authorization from employees before ordering drug tests or medical exams. Doctors Place is not liable for any claims arising from an employer's failure to comply with applicable laws regarding workplace drug testing or medical examinations.

You may not reproduce, distribute, or create derivative works from any content on this Service without our express written permission.
THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED. DOCTORS PLACE DISCLAIMS ALL WARRANTIES, INCLUDING BUT NOT LIMITED TO IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.
We do not warrant that the Service will be uninterrupted, error-free, or free of viruses or other harmful components. We do not warrant the accuracy, completeness, or timeliness of any content on the Service.
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, DOCTORS PLACE INC., ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES — INCLUDING LOSS OF PROFITS, DATA, OR GOODWILL — ARISING OUT OF OR IN CONNECTION WITH YOUR USE OF THE SERVICE OR THESE TERMS, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
OUR TOTAL LIABILITY TO YOU FOR ANY CLAIMS ARISING OUT OF OR RELATED TO THESE TERMS OR THE SERVICE SHALL NOT EXCEED THE AMOUNT YOU PAID TO DOCTORS PLACE IN THE 12 MONTHS PRECEDING THE CLAIM.
You agree to indemnify, defend, and hold harmless Doctors Place Inc. and its officers, directors, employees, and agents from and against any claims, liabilities, damages, losses, and expenses (including reasonable attorneys' fees) arising out of or related to your use of the Service, your violation of these Terms, or your violation of any applicable law or regulation.
These Terms are governed by the laws of the State of New Jersey, without regard to its conflict of law provisions. Any dispute arising from these Terms or your use of the Service shall be resolved exclusively in the state or federal courts located in Bergen County, New Jersey.
We reserve the right to modify these Terms at any time. We will notify you of material changes by posting the updated Terms on this page with a new effective date. Your continued use of the Service after changes are posted constitutes your acceptance of the revised Terms.
We reserve the right to suspend or terminate your access to the Service at any time, for any reason, including violation of these Terms, without notice or liability.
