What Is a DOT Drug Test?

A DOT drug test is a federally mandated urine drug screen required for safety-sensitive employees in the transportation industry β€” including CDL truck drivers, bus drivers, airline workers, railroad employees, and pipeline workers. The test is governed by 49 CFR Part 40 and administered by FMCSA, FAA, FTA, FRA, PHMSA, or USCG depending on your industry.

What Does a DOT Drug Test Screen For?

The DOT 5-panel test screens for the following substances:

  • Marijuana (THC) β€” Federal standard applies regardless of state law. Even in legal states, a positive THC result will disqualify you.
  • Cocaine
  • Amphetamines β€” Including methamphetamine, MDMA (ecstasy), and prescription amphetamines (Adderall, etc.)
  • Opiates β€” Including heroin, codeine, morphine, and hydrocodone
  • Phencyclidine (PCP)

Note: The DOT drug test does NOT screen for alcohol. Alcohol testing is done separately via a Breath Alcohol Test (BAT).

When Is a DOT Drug Test Required?

  • Pre-employment β€” Required before a driver can operate a CMV for a new employer.
  • Random β€” Employers must randomly test a percentage of their workforce throughout the year (currently 50% for drugs, 10% for alcohol).
  • Post-accident β€” Required after a fatal accident; also required for accidents involving injury or vehicle tow-away depending on fault.
  • Reasonable suspicion β€” When a supervisor has documented cause to believe a driver is impaired.
  • Return-to-duty β€” After a violation, before returning to safety-sensitive functions.
  • Follow-up β€” Unannounced testing for a minimum of 12 tests in the first 12 months after return-to-duty.

Step-by-Step: What Happens at the Collection Site

  1. Check in β€” Show a valid photo ID. The collector verifies your identity.
  2. Empty pockets and wash hands β€” You'll be asked to leave bags, coats, and personal items outside the restroom.
  3. Provide a specimen β€” You collect at least 45 mL of urine in a private restroom. The collector may or may not directly observe the collection depending on circumstances.
  4. Specimen is sealed β€” The collector checks temperature (within 2 minutes of collection) and initiates a Federal Chain of Custody (CCF) form.
  5. Split specimen β€” The specimen is split into Bottle A (primary) and Bottle B (split), both sealed and labeled in your presence.
  6. Shipment to lab β€” Specimens are shipped to a SAMHSA-certified laboratory.

How Are Results Processed?

The lab runs initial immunoassay screening. Any positive or anomalous result triggers a confirmatory GC-MS (gas chromatography–mass spectrometry) test. All results β€” positive or negative β€” are reviewed by a licensed Medical Review Officer (MRO) before being reported to the employer.

Negative results are typically available in 24–48 hours. Positive results or specimens requiring additional review can take up to 5–7 business days.

What Happens If You Test Positive?

The MRO will contact you before reporting a positive result to give you an opportunity to explain a legitimate medical reason (e.g., a valid prescription). If no legitimate explanation exists, the positive is reported to your employer and the violation is entered into the FMCSA Drug and Alcohol Clearinghouse.

A positive result means immediate removal from safety-sensitive functions. To return to work, you must complete a Substance Abuse Professional (SAP) evaluation, complete any recommended treatment, and pass a return-to-duty test.

Can You Request a Retest?

Yes. If you test positive, you may request the Bottle B (split) specimen be tested at a second SAMHSA-certified lab within 72 hours of MRO notification. This is at your expense.
