Who needs it: All commercial drivers holding a CDL are required by the FMCSA to pass a DOT physical to maintain their driving eligibility. This includes truck drivers, bus drivers, delivery drivers operating vehicles over 10,001 lbs, and drivers transporting hazardous materials.

What it covers: A comprehensive physical examination tailored to the job requirements, including musculoskeletal assessment, cardiovascular screening, vision and hearing, and functional capacity testing depending on the role.
Why employers use it: Reduces workers compensation exposure, confirms fitness for duty, and documents a health baseline before employment begins — particularly important for jobs with heavy lifting, machinery operation, or safety-sensitive duties.
How to order: Employers can create a team account at portal.dot-physical.net and order for individual employees or entire new hire classes. Results are routed back to the employer dashboard.
Who needs it: DOT-regulated employers are federally required to conduct pre-employment, random, post-accident, reasonable suspicion, and return-to-duty drug tests for safety-sensitive employees. Non-DOT employers use drug screening for pre-employment clearance and workplace safety policies.
DOT 5-Panel Urine Drug Screen — $69.99: Tests for marijuana, cocaine, opiates, amphetamines, and PCP. Collected by a certified collector at any of our 15,000+ sites using strict chain-of-custody procedures required by FMCSA. Results typically within 24-72 hours.

Random Testing Consortium — $49/year per driver: DOT-regulated employers with fewer than 50 safety-sensitive employees are required to participate in a consortium. Work OccMed manages your random pool, handles FMCSA Clearinghouse reporting, and notifies you when an employee is selected.
Who needs it: Healthcare workers, school employees, childcare staff, laboratory workers, and others in patient-facing or high-exposure roles are often required to prove immunity to specific diseases before starting work. Titer testing verifies that immunity exists — either from prior vaccination or prior infection.

What happens: A blood draw is collected at a certified site. The sample is analyzed at a certified lab. Results are returned to you or your employer, typically within 3-5 business days.
Who needs it: OSHA requires fit testing for any employee who wears a tight-fitting respirator on the job. This includes N95 masks, half-face respirators, full-face respirators, and supplied-air respirators. Common industries include healthcare, construction, industrial manufacturing, emergency response, painting, and chemical handling.
What it tests: That the specific make, model, and size of respirator creates an adequate seal on the wearer's face. A poor fit means the respirator does not protect — even if worn correctly.
How often: Required annually, when a new respirator model is assigned, or when the wearer has significant changes to facial structure such as weight loss or dental surgery.
Test types: Qualitative fit testing (QLFT) uses a challenge agent the wearer can detect if the seal breaks. Quantitative fit testing (QNFT) uses instrumentation to measure actual particle penetration. Both are OSHA-accepted for most respirator types.
Who needs it: DOT-regulated employers are required to conduct alcohol testing for safety-sensitive employees pre-employment, post-accident, and upon reasonable suspicion. Non-DOT employers use BAT for workplace alcohol policies.
DOT thresholds: A BAC of 0.02 or above requires removal from safety-sensitive duties. A BAC of 0.04 or above is a DOT violation requiring SAP evaluation before return to duty.
Who needs it: Healthcare workers, corrections employees, laboratory workers, and others in high-exposure environments are commonly required to complete annual TB testing. The PPD (Mantoux tuberculin skin test) is the standard screening method.
How it works: A small amount of tuberculin is injected under the skin of the forearm. The site is read 48-72 hours later. A positive result requires follow-up with a chest X-ray and clinical evaluation.
