What to Expect at Your DOT Physical Exam

Why DOT Physicals Exist

The Federal Motor Carrier Safety Administration (FMCSA) requires every commercial driver operating a vehicle over 10,001 lbs, carrying hazardous materials, or transporting 9+ passengers to hold a valid Medical Examiner's Certificate. The DOT physical ensures that drivers are physically and mentally fit to safely operate large vehicles on public roads.

What the Exam Covers

A DOT physical is a comprehensive medical evaluation. The certified medical examiner (CME) will review:

  • Vision — Distant acuity of at least 20/40 in each eye (with or without correction). Full peripheral field of at least 70° in the horizontal meridian.
  • Hearing — Must perceive a forced whispered voice in the better ear at a distance of not less than 5 feet.
  • Blood pressure & pulse — Hypertension is one of the most common reasons for short-term or conditional certification.
  • Urinalysis — Screens for diabetes (glucose), kidney disease (protein), and other conditions. This is not a drug test.
  • Physical examination — Head, eyes, ears, mouth, throat, cardiovascular, lungs, abdomen, spine, extremities, and neurological function.
  • Medical history review — Any conditions such as sleep apnea, seizures, insulin-dependent diabetes, or cardiac history can affect certification.

How to Prepare

  1. Bring a list of all current medications including dosages.
  2. If you wear glasses or contacts, bring them — along with any prescription paperwork if available.
  3. If you use a CPAP machine for sleep apnea, bring your compliance report showing usage for the past 90 days.
  4. Bring any specialist letters or clearances for known conditions (cardiac, neurological, etc.).
  5. Get adequate rest the night before — fatigue can temporarily elevate blood pressure.

Common Disqualifying Conditions

Certain conditions are per-se disqualifying under 49 CFR §391.41, including:

  • Insulin-dependent diabetes (without an exemption)
  • Vision worse than 20/40 in either eye that cannot be corrected
  • Loss of a foot, leg, hand, or arm (without an exemption)
  • History of epilepsy or seizure disorders (without a sufficient seizure-free period)
  • Active substance abuse or dependence
  • Established history of cardiovascular disease that is likely to cause sudden incapacity

Many conditions that are not per-se disqualifying are evaluated on a case-by-case basis at the examiner's discretion.

Certification Periods

A clean exam typically earns a 2-year certificate. Drivers with certain conditions (like controlled hypertension) may receive a shorter certification — 1 year or even 3 months — requiring more frequent monitoring. Drivers who miss their renewal date have an expired CDL medical certificate and cannot legally operate a CMV until they recertify.

Yes. Blood pressure of 180/110 or higher (Stage 3 hypertension) will result in disqualification until it is controlled. Stage 2 hypertension (160–179/100–109) earns a 3-month temporary certificate. Stage 1 (140–159/90–99) earns a 1-year certificate instead of the standard 2-year.
Does a DOT physical include a drug test?+
No. The DOT physical urinalysis screens for medical conditions like diabetes and kidney disease — it is not a drug test. DOT drug testing (urine screen for the federal 5-panel) is a separate process ordered by your employer.
